Affinity Health Management in Danbury, CT, is seeking a compassionate hospice aide to work under the supervision of a registered nurse, delivering PERSONAL care to meet patients' needs and promote comfort. You will document observations and report care performed, contributing to safety and quality across the patient’s plan of care.

Responsibilities include bathing, dressing, feeding, and assisting with mobility, ROM, and light housekeeping.

Competenze

• Completion of state approved CNA course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on).
• Current Food Handlers Permit.
• High school diploma or GED.
• Able to read and follow written instructions and document care.
• Understands hospice philosophy and terminally ill care.
• Prior hospice/long-term care or equivalent experience preferred.
• Basic computer skills to document in EMR.

Mansioni

• Provide personal care and attend to patient requests promptly.
• Communicate with patient and family with confidentiality and respect.
• Ensure safety and proper use of equipment (foot stools, side rails, O2).
• Assist with bathing, transfers, dressing, and feeding as assigned.
• Document observations and report changes to RN Case Manager.
• Assist with ambulation, ROM, and light homemaker tasks.
