Job summary

Amedisys, part of the Optum family, is seeking a CNA Hospice Aide to provide compassionate, hands-on care to patients and support for their families during end-of-life care. This role focuses on assisting with daily living activities while promoting comfort, dignity, and emotional well-being within the home setting.

You will work closely with the hospice care team to ensure patients receive respectful, individualized care in alignment with the plan of care.

Qualifications

• Valid driver's license, reliable transportation and liability insurance.
• Current state CNA certification/registration.
• Current CPR certification.

Responsibilities

• Provides personal care services to patients
• Assists with ambulation, transfers, ROM, positioning and household tasks
• Plans and prepares nutritious meals and assists with feeding
• Performs homemaking and environmental services
• Observes and reports changes in patient status to nurse
• Follows written care instructions from a registered nurse
• Assists with prescribed exercises as instructed by nurse or therapist
• Performs and records simple urine tests for sugar, acetone, and albumin
• Other duties as assigned
