Job summary

Bristol Hospice is seeking Certified Home Health Aides to join our compassionate team. You will work under the supervision of a Registered Nurse and provide personal care, hygiene, mobility assistance, and meal support in patients' homes.

The ideal candidate is a CNA in good standing with current HHA certification and CPR, with a preference for hospice or community health experience. A driver’s license and reliable vehicle are required.

Qualifications

• Must be a CNA in good standing.
• Current HHA certification (CA) in good standing if the state requires it.
• Preferred 1 year experience in hospice or community health.
• Licensed driver with insured vehicle and valid license.
• Current CPR certification.

Responsibilities

• Provide personal care and attend to patient requests.
• Provide positive communication to patient and family with confidentiality.
• Meet safety needs and use equipment properly.
• Assist in dressing, bathing, meals, and mobility.
• Take and record vital signs as ordered.
• Assist with ambulation and range of motion.
• Document care activities and report changes to the Case Manager.
