Job summary

Bristol Hospice is seeking a compassionate Certified Home Health Aide (CHHA) to join our interdisciplinary team in home hospice settings. You will work under the supervision of a registered nurse, providing personal care, monitoring patients, and supporting families with dignity and respect.

The ideal candidate is a currently licensed CNA, CPR certified where required, and able to communicate with empathy while maintaining confidentiality.

Qualifications

• Must hold an active CNA license in good standing.
• CPR certification preferred or required per state.
• Ability to read and follow care plans and physician orders.

Responsibilities

• Provide personal care and support to patients in home hospice settings.
• Assist with daily activities: bathing, grooming, toileting, nutrition.
• Monitor patient condition and report changes to RN Case Manager.
• Maintain safety and hygiene, use equipment properly.
• Support families with education and compassionate communication.
