Job summary

Bristol Hospice is seeking an experienced Certified Home Health Aide (CHHA) to join our compassionate team. You will work under RN supervision to meet personal care needs and support comfort for patients in their homes.

The ideal candidate is a CNA in good standing with a California HHA certification, with 1+ year in hospice or community health. A valid driver’s license and the ability to pass annual background checks are preferred.

Qualifications

• Must be a CNA in good standing; CA requires HHA in good standing.
• Preferred 1 year experience in community health, hospice, or medical/surgical settings.
• Licensed driver with automobile insured in accordance with organization/state requirements.

Responsibilities

• Provide personal care and attend to patient requests, reporting to the Case Manager if needed.
• Provide compassionate communication with patients and families, maintaining confidentiality and respect.
• Meet safety needs and use equipment properly (foot stools, side rails, O2, etc.).
• Assist with baths, back rubs, oral hygiene, hair care, and changing bed linens as assigned.
• Assist in dressing, undressing, meals, and feeding as assigned.
• Take and record basic vitals when ordered and demonstrate competency.
• Monitor skin condition and report changes to the Case Manager.
• Assist with ambulation, ROM, and transfers as directed.
