Job summary

Bristol Hospice is seeking an experienced Certified Home Health Aide (CHHA) to join our compassionate team serving patients in home-based hospice care. You will work under the supervision of a Registered Nurse, providing personal care, support with daily activities, and essential comfort measures to patients and families.

The ideal candidate is a licensed CNA with CPR certification, a valid driver's license, and a caring, patient-centered approach.

Qualifications

• Must be a Certified Nursing Assistant (CNA).
• Completed a minimum of 75 hours of classroom and supervised practical training.
• Licensed driver with automobile insured in accordance with requirements.
• Willingness to submit to a criminal background check.
• Current CPR certification required.
• Hospice/home health experience preferred.

Responsibilities

• Provide personal care and attend to patient requests.
• Communicate with patient/family with empathy and confidentiality.
• Meet safety needs and use equipment properly.
• Assist with bathing, grooming, and dressing.
• Plan and assist with nutritious meals as assigned.
• Record vital signs when ordered and report changes.
• Assist with mobility and transfers as directed by care team.
• Support medication reminders as allowed by state rules.
• Maintain a clean and organized patient living area.
• Adhere to care plans and documentation procedures.
