Qualifications

• Completion of a state approved CNA course and current license.

• Hospice experience preferred.

Responsibilities

• Perform personal care and assist with activities of daily living.
• Communicate with patients and families with empathy and confidentiality.
• Ensure safety and proper use of equipment.
• Assist with feeding, bathing, grooming, and changing linens.
• Document observations and care provided in the EMR.
• Report changes to the RN Case Manager.
