Job summary

Continuum Care Hospice in the Pacific Northwest is seeking a compassionate hospice aide to support patient comfort under RN supervision, performing tasks per the patient’s plan of care.

The aide will provide personal care, assist with activities of daily living, communicate with families, ensure safety, and document care. CNA certification and a reliable vehicle are typically required for this role.

Qualifications

• Completion of CNA course with current license.
• Ability to read and follow written instructions.
• Comfort with hospice philosophy and terminally ill care.
• Prior home health/hospice experience preferred.

Responsibilities

• Provide personal care and support per patient plan of care.
• Communicate compassionately with patients and families.
• Ensure safety and proper use of equipment during care.
• Assist with bathing, grooming, and activities of daily living.
• Support feeding and meal preparation as directed.
• Document observations and care provided in EMR.
• Report changes in patient condition to RN Case Manager.
• Assist with mobility, transfers, and range of motion.
