Job summary

Affinity Hospice is seeking a compassionate hospice aide (CNA) to work under RN supervision and support patients in their homes. Responsibilities include personal care, observation and timely reporting of changes, and documentation of care performed to promote patient comfort and dignity.

Hours are full-time (40 hours/week) with travel to patient homes, flexible scheduling, and potential for weekend rotations.

Qualifications

• Meet state requirements for CNA and training certification.
• Possess CPR certification and valid driver’s license with insured vehicle.
• Ability to lift up to 50 lbs and assist with patient transfers.
• Demonstrate empathy for ill and frail patients.

Responsibilities

• Provide personal care and assist with activities of daily living.
• Observe and report patient condition changes to the case manager.
• Follow care plans and documentation standards in patient records.
• Assist with feeding, bathing, grooming and mobility as assigned.
