Job summary

Affinity Hospice is seeking a dedicated Certified Nursing Assistant to support patients in their homes under the supervision of a registered nurse. You will perform personal care tasks, monitor patient comfort, and document observations to help maintain quality care.

Candidates should meet state training requirements, hold an active CPR certification, and have a valid driver’s license with reliable transportation.

Qualifications

• Meet the training and certification requirements of the State.
• Provide compassionate personal care and observe patient condition.
• Document care performed and report changes to case manager.

Responsibilities

• Assist with activities of daily living (bathing, grooming, toileting).
• Assist in feeding and ensuring patient comfort.
• Report observations and changes to the nurse and case manager.
• Follow safety procedures and use equipment properly.
