Job summary

Bristol Hospice is seeking a Certified Home Health Aide (CHHA) to join our Des Plaines team in Illinois. This role involves performing personal care, assisting with daily living activities, and delivering compassionate, patient-centered hospice care under the supervision of a registered nurse.

The ideal candidate is a licensed CNA with CPR certification, a valid driver’s license, and the ability to work autonomously in patients’ homes.

Qualifications

• Must be a Certified Nursing Assistant (CNA).
• Preferred to have one year of experience in community health/hospice or medical/surgical environments.
• Must have completed a minimum of 75 hours of classroom and supervised practical training.
• Licensed driver with automobile that is insured and in good working order.
• Willingness to submit to a criminal background check prior to hire and annually thereafter.
• Current CPR certification.

Responsibilities

• Perform personal care needs after a patient’s assignment and attend to requests promptly; report to the Case Manager if unable to perform a task.
• Provide patient and family with positive communication, within ethical and respectful care, including confidentiality.
• Meet safety needs of patient and use equipment safely.
• Assist in dressing, bathing, meals, feeding, and changing bed linen as assigned.
• Assist with ambulation and exercise as instructed by the hospice nurse or therapist.
• Support family/caregiver with respite when on-site, as appropriate.
• Maintain patient living area clean and orderly; adhere to care procedures and professional conduct.
