Job summary

Bristol Hospice is seeking an experienced Certified Home Health Aide (CHHA) to join our team and support patients and families in the Madison area. Under RN supervision, you will provide essential personal care and compassionate, privacy-respecting services in patients' homes.

The role emphasizes autonomy, strong communication, safety, and adherence to care plans, with opportunities for advanced training and meaningful work in hospice care.

Qualifications

• Must be a CNA in good standing; California residents must also hold an HHA credential.
• Preferred 1 year of hospice or community health experience.
• Licensed driver with insured vehicle and valid license.
• Willingness to undergo background checks and CPR certified.

Responsibilities

• Provide personal care and attend to patient requests promptly.
• Communicate with patient and family respectfully and confidentially.
• Meet safety needs and use equipment properly (e.g., foot stools, side rails).
• Assist with dressing, bathing, meals, and nutrition as assigned.
• Take and record vital signs when ordered.
• Assess patient condition and report changes to Case Manager.
• Assist with ambulation, transfers, and range of motion as directed.
• Assist with medications as allowed by state regulations.
• Maintain patient living areas clean and orderly; provide respite when appropriate.
• Adhere to documentation and care procedures; participate in QA activities.
• Respect patient environment and personal needs; perform other duties as assigned.
