Job summary

Bristol Hospice in Chicago, IL is seeking a Certified Home Health Aide (CHHA) to join our Interdisciplinary Group Team. You will work under the supervision of a Registered Nurse and provide personal care to patients in their homes, promoting comfort and dignity.

This full-time, hourly position offers $20.00-$25.00 per hour and requires CNA certification, CPR, and a valid driver’s license with a reliable vehicle.

Qualifications

• Must be a Certified Nursing Assistant.
• One year of experience in community health, hospice, or similar setting preferred.
• Minimum 75 hours of classroom and supervised practical training.
• Licensed driver with insured vehicle in good working order.
• Willingness to undergo criminal background checks and CPR certification.

Responsibilities

• Perform personal care and attend to patient requests promptly.
• Provide patient and family with compassionate communication and confidentiality.
• Assist with meals, grooming, and daily living activities.
• Monitor and document patient condition, report changes to Case Manager.
• Support transfers, ambulation, and medication reminders as allowed.
