Job summary

Patriot Healthcare in Pickens, SC is seeking a Hospice Certified Nursing Assistant to provide personal care to assigned patients in accordance with nursing assessments and the care plan under the supervision of a nurse.

You will participate with the Interdisciplinary Group in developing and implementing the patient/family plan of care, document tasks, communicate with staff and family, and help ensure patient comfort and dignity in end‑of‑life care.

Responsibilities

• Stewards the dignity and respectful response to patients with terminal or chronic illnesses.
• Participates with the IDG in care plan development and implementation.
• Performs tasks per hospice policies and supervisor instructions.
• Ensures patient comfort and reports changes to the nurse.
• Documents visits and communicates with family and staff.
• Maintains warm, calm environment and supports patient dignity.
• Reports concerns to supervisors and addresses complaints.
