Job summary

Responsibilities

• Participates with the IDG to develop and implement the patient/family care plan.
• Performs tasks per hospice policies and supervisor instructions.
• Assists patients with daily activities such as mouth care, baths, dressing, hair care, and grooming.
• Assists with mobility and transfers, positioning, and use of assistive devices.
• Documents care and other comments related to visits.
• Communicates with family or facility staff to coordinate nursing services.
• Creates a warm, calm environment and maintains open communication with the supervisor.
• Reports complaints and grievances from patients or caregivers.
