Job summary

Patriot Healthcare seeks a Hospice Certified Nursing Assistant (CNA) to deliver compassionate, patient-centered personal care in alignment with nursing assessments and the care plan. You will work under nurse supervision to support comfort, dignity, and overall well-being in a hospice setting.

You will collaborate with the interdisciplinary group to implement the care plan, document tasks, and maintain clear communication with patients, families, and facility staff.

Qualifications

• CNA certification or license required.
• Experience in hospice or end-of-life care preferred.
• Ability to communicate with patients and families.

Responsibilities

• Provide personal care services to assigned patients per nursing assessments and care plan.
• Participate with the IDG in developing and implementing the patient/family’s plan of care.
• Follow Hospice policies and procedures and instructions from supervisor.
• Assist with daily living activities to keep patient as comfortable as possible.
• Document tasks performed and report to supervisor; communicate with family when appropriate.
• Create a warm, calm environment and show compassionate care for patients and families.
• Report any concerns or changes in patient condition to the supervisor.
