Affinity Hospice Management LLC in Port Saint Lucie, FL seeks a hospice aide to support patients under RN supervision. You will perform personal care, assist with daily activities, monitor patient condition, and report changes to the case manager.

This role emphasizes safety, confidentiality, and compassionate, patient-centered care. Responsibilities include assisting with bathing, feeding, mobility, and basic documentation in EMR, while ensuring a clean and safe living environment for patients

Formación

• Completion of state-approved CNA course or equivalent.
• Certified Nursing Assistant (NAC) required.
• Current CPR certification (hands-on required).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and provides compassionate care.
• Prior home health/hospice or related LT care experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Responsabilidades

• Perform personal care tasks as assigned and promptly attend to patient requests.
• Provide patient and family with compassionate communication and confidentiality.
• Meet safety needs and use equipment safely (foot stools, side rails, O2, etc.).
• Provide baths, back rubs, oral hygiene, shampoos, and linen changes.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding while avoiding homemade foods from staff.
• Observe and report skin condition to prevent tissue breakdown.
• Report pain or changes in condition to RN Case Manager.
• Assist in ambulation and ROM as instructed.
• Provide normal ROM and transfers/positioning as directed.
• Support self-administered medications reminders as allowed by state regs.
• Assist with bedpans/urinals and light housekeeping tasks.
• Perform ROM and simple procedures with competency.
• Provide respite for patient/family/caregiver when on-site.
• Keep living area clean and orderly; follow care procedures and standards.
• Participate in quality assessment and performance improvement activities.
• Respect patient and family environment and personal needs.
• As mandatory reporter, report non-compliance to Dept. of Health within 14 days.
• Report suspected abuse or neglect to proper authorities immediately.
