Job summary

Qualifications

• Completion of state-approved CNA course or equivalent training.
• NAC certification completed and current.
• CPR certification current and hands-on demonstrated.
• Food Handlers Permit valid.
• High school diploma or GED obtained.
• Able to read and follow written instructions and document care.

Responsibilities

• Assist with personal care and daily activities as assigned.
• Communicate with patient and family respectfully and confidentially.
• Ensure safety and proper use of equipment.
• Provide baths, oral hygiene, and linen changes as directed.
• Assist in dressing, feeding, and mobility as instructed.
• Report pain or changes to the RN Case Manager promptly.
• Support self-administered medications as allowed by state rules.
• Maintain clean, orderly living area and perform light homemaker tasks.
• Document observations and participate in quality improvement activities.
• Adhere to organization procedures and professional standards.
