Qualifications

• State approved CNA course or equivalent training.
• NAC certification is required.
• CPR certification current (hands-on demonstration required).
• Food handlers permit current.
• HS diploma or GED obtained.
• Able to read/write and document care.
• Hospice mindset and terminally ill care understanding.
• 1 year hospice/home health experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years old.
• Licensed driver with insured vehicle.

Responsibilities

• Provide personal care as assigned; report gaps to Case Manager.
• Communicate with patient/family with confidentiality and respect.
• Ensure safety and proper use of equipment.
• Assist with baths, grooming, hygiene, and linen changes.
• Assist in dressing, feeding, and mobility as directed.
• Maintain skin care to prevent tissue breakdown.
• Report pain or changes to RN Case Manager.
• Assist with transfers and range of motion as instructed.
• Support self-administration reminders for medications where allowed.
• Help with light homemaker tasks and bedpan/urinal use.
