Job summary

Affinity Health Management seeks a compassionate Hospice Aide to support patients under the RN's supervision, delivering personal care and comfort per care plan. You will report observations, assist with daily living activities, and communicate with patients and families with empathy.

Responsibilities include bathing, dressing, mobility assistance, basic wound care, and documentation in EMR. Must hold CNA and NAC, CPR, and a valid driver’s license; 18+ years old with high school diploma or GED.

Qualifications

• Completion of a state-approved CNA course or equivalent
• NAC certification is required
• CPR certification, hands-on proof required
• High school diploma or GED or equivalent
• Ability to read and follow written instructions and document care given
• Prior hospice/home health experience preferred
• Basic computer skills to document in EMR
• Must be at least 18 years old
• Licensed driver with valid vehicle and insurance

Responsibilities

• Provide personal care including baths, grooming, and dressing
• Assist with meals, ambulation, transfers, and mobility
• Monitor patient condition and report changes to RN Case Manager
• Maintain safety and infection control practices
• Document observations and care provided in EMR
• Respect patient privacy and family dynamics while delivering care
