Job summary

Affinity Health Management is seeking a compassionate Hospice Aide to support patients under RN supervision. You will perform personal care, monitor comfort, report observations, and help maintain a clean, safe environment according to the plan of care.

Responsibilities include assisting with bathing, dressing, feeding, transfers, and documentation in EMR; upholding confidentiality; and cooperating with the care team to ensure quality end-of-life support.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Certified Nursing Assistant (CNA) and NAC credentials are required.
• Current CPR certification with hands-on demonstration required.
• Food handlers permit valid for serving food safety compliance.
• High school diploma or GED is required.
• Ability to read and follow written instructions and document care given.
• Understanding hospice philosophy and care for terminally ill patients.
• One year hospice/home health experience preferred.
• Basic computer skills to document in EMR.

Responsibilities

• Provide personal care per patient plan and report to Case Manager.
• Communicate with patient and family with empathy and confidentiality.
• Ensure safety and proper use of equipment and supplies.
• Assist with bathing, dressing, and grooming as assigned.
• Assist in feeding and position transfers as instructed.
• Document observations and care in EMR as required.
• Report changes in patient condition to RN Case Manager.
• Maintain clean living area and assist with light housekeeping.
