Job summary

Affinity Health Management is seeking a hospice aide to support patients under the supervision of a registered nurse. You will perform personal care, document observations, and promote comfort per the patient’s plan of care.

Responsibilities include assisting with bathing, dressing, feeding, and activities of daily living while ensuring safety and confidentiality. A valid CNA, CPR, and COVID vaccination are required, with a focus on quality, continuity, and compassionate care.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Covid vaccination is required due to multiple facilities and guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable providing specialized care to the terminally ill.
• Prior experience in hospice, orderly, acute, rehabilitative, or long term care facility; 1 year preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with automobile that is insured and in good working order.

Responsibilities

• Performing personal care needs after given a patient's assignment and attending to requests promptly.
• Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.
• Meeting safety needs of patient and using equipment safely and properly.
• Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as assigned.
• Assisting in dressing and undressing patients as assigned.
• Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items.
• Providing proper care and observation of patient's skin to prevent tissue breakdown.
• Reporting pain or other symptoms to the RN Case Manager; aware of caregiver/interpersonal issues.
• Assisting in ambulation and exercise as instructed by the hospice nurse or therapist.
• Provide range of motion and transfers/positioning as directed.
• Assistance with self-administered medications as allowed by state regulations, such as reminders.
• Offering and assisting with bedpans and urinals; light laundry and waste tasks.
• Performing range of motion and simple procedures as ordered with skills competency.
• Providing respite for patient/family/caregiver when on-site, as appropriate.
• Keeping patient's living area clean and orderly as assigned.
• Adhering to documentation and care procedures and standards of conduct.
• Participates in quality assessment performance improvement activities.
• Respectful of patient and family/caregiver environment and personal needs.
• As a mandatory reporter, reports to authorities as required by law within 14 days.
• Reports suspected abuse or neglect to the proper authorities; reports must be submitted immediately when required.
