Job summary

Affinity Health Management in Florida is seeking a hospice aide to provide compassionate personal care under RN supervision, following the patient’s plan of care and documenting observations.

Responsibilities include bathing, dressing, feeding assistance, safety, reporting changes to the RN, and maintaining cleanliness. Must have CNA training, NAC Florida, CPR, HS diploma, and a valid driver’s license; hospice experience preferred; EMR documentation skills required.

Qualifications

• Completion of a state approved CNA course or equivalent training.
• Nursing Assistant Certification (NAC) in Florida.
• Current Healthcare Provider CPR certification.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Prior hospice or long-term care experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age and valid driver with insured vehicle.

Responsibilities

• Perform personal care tasks and respond to patient requests promptly.
• Provide compassionate communication with patient and family.
• Ensure safety and proper use of equipment.
• Assist with bathing, dressing, feeding and hygiene.
• Support ambulation and transfers as directed by the RN.
• Document observations and report changes to the RN Case Manager.
• Maintain clean, orderly living areas and adhere to care standards.
• Protect confidentiality and comply with regulations.
