Job summary

Affinity Health Management is seeking a Hospice Aide to support patient comfort under RN supervision. Responsibilities include personal care, mobility assistance, documenting observations, and ensuring safety.

You will engage with patients and families with compassionate communication while maintaining confidentiality and following the plan of care. The role requires a state-approved CNA course, Florida NAC certification, CPR, and a high school diploma or GED, with prior hospice or home health

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC) in the state of Florida.
• Current Healthcare Provider CPR certification (hands-on, not internet-based).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and comfort providing specialized care to the terminally ill.
• 1 year hospice/home health/long-term care experience preferred.
• Basic computer skills to document in EMR.

Responsibilities

• Perform personal care tasks and attend to patient requests promptly.
• Provide positive communication with patient and family while maintaining confidentiality.
• Ensure safety and proper use of equipment.
• Assist with baths, oral care, grooming, and changing bed linens.
• Assist in dressing, undressing, and feeding as assigned.
• Support patient with mobility, ROM exercises, and transfers.
• Observe and report pain or changes to the RN Case Manager.
• Document care and observations in EMR as required.
• Assist with light housekeeping and meal-related tasks as needed.
• Adhere to organizational policies and quality improvement activities.
