Qualifications

• High school diploma or equivalent required.
• Certified Nursing Assistant (CNA) in good standing on the Ohio Department of Health Nurse Aide Registry required.
• Two years experience as CNA/home health aide preferred.

Responsibilities

• Provide personal care per the plan of care (bathing, hygiene, grooming).
• Observe and report changes in patient condition to RN Case Manager.
• Assist with mobility and positioning; support family and patient needs.
• Provide and document care using the electronic medical record.
• Maintain a safe, clean environment and comply with regulations.
• Rotate weekends, holidays, and on-call shifts as needed.
