Job summary

Affinity Hospice Management LLC is seeking a Hospice Aide to work under the RN supervisor, delivering direct patient care as part of a supportive team in a hospice setting.

The role requires CNA/NAC credentials, CPR, and a compassionate approach to terminally ill patients. You will document care in EMR, assist with activities of daily living, and participate in safety-focused tasks to promote comfort and quality of life.

Qualifications

• Completion of a state approved CNA course or equivalent training.
• NAC certification is required.
• Current Healthcare Provider CPR certification (hands-on validation).
• Current Food Handlers Permit is required.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable caring for terminally ill.
• 1 year hospice/home health/long-term care experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care needs as assigned and promptly respond to requests.
• Provide patient and family with positive communication while maintaining confidentiality.
• Ensure safety and proper use of equipment.
• Assist with bathing, hygiene, nail care, and linen changes.
• Assist in dressing, undressing, and feeding as assigned.
• Offer bedpan/urinal support; light housekeeping and laundry as needed.
• Monitor skin and report changes to RN Case Manager.
• Assist in mobility, transfers, and range of motion as directed.
• Document observations and care performed per procedures.
• Report any concerns about abuse or neglect immediately.
