Responsibilities

• Provide direct patient care under the RN and Plan of Care.
• Assist with safe transfers and ambulation per Hospice policy.
• Report changes and document care to ensure continuity of care.
• Practice infection control; maintain a clean, safe environment.
• Communicate respectfully with patient, family, and staff.
• Assist with nutrition and fluid intake for the patient.
