Qualifications

• High School Diploma preferred.
• A) Training program/competency evaluation per Medicare §418.76.
• B) Competency evaluation program per §418.76.
• C) Nurse aide training/competency evaluation program approved by state.
• D) State licensure program per §418.76(b).

Responsibilities

• Provides direct patient care under RN direction.
• Assists with safe transfers and ambulation per Hospice policy.
• Reports changes and documents care rendered for continuity of care.
• Maintains infection control, a clean safe environment.
• Promotes respectful communication with patient/family and staff.
• Respects patient privacy and property.
• Assists with proper nutrition and fluid intake.
• Manages time effectively for Hospice operations.
• Performs treatments and procedures according to Hospice guidelines.
