Job summary

Valor Hospice is seeking a Hospice Aide to provide direct patient care under RN supervision in patients’ homes or residence. This role emphasizes safe transfers, accurate reporting, infection control, and maintaining a clean, respectful care environment.

The ideal candidate will have a high school diploma (preferred) and completed nurse aide training or equivalent, plus a valid driver’s license and at least one year of relevant experience. Reliable transportation is needed.

Qualifications

• Education: High School Diploma preferred.
• Completion of a Medicare/Medicaid recognized nursing aide program or state approved training.
• Must have a current driver’s license.
• Experience: 1 year as Hospice Aide or Nursing Assistant preferred.
• Ability to read, write and report clinical information clearly.

Responsibilities

• Provides direct patient care under RN direction and Plan of Care.
• Assists with safe transfers and ambulation per Hospice policy.
• Documents care and reports changes to ensure continuity of care.
• Follows infection control principles.
• Maintains a clean, safe and comfortable environment.
• Promotes respectful communication with patient, family and staff.
• Respects patient privacy and property.
• Assists with nutrition and fluid intake as needed.
• Supports efficient hospice operations with good time management.
• Performs treatments and procedures per Hospice guidelines.
