Job summary

Qualifications

• Knowledge and understanding of hospice philosophy.
• Strong oral and written communication skills.
• Ability to handle death/dying scenarios with sensitivity.
• Ability to work independently and manage tasks.
• Ability to communicate with facility staff when assigned to facility-based care.
• Comfort with hand-held devices and EHR/documentation.
• Demonstrates empathy for ill, injured, frail and impaired patients.

Responsibilities

• Provide personal care and assist with daily living activities per plan of care.
• Maintain patient safety and comfort, use equipment correctly.
• Assist with meals, hydration and basic routine tasks.
• Document care provided and report changes to supervising nurse.
• Support patient and family respite needs during on-site care.
