Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Current Food Handlers Permit.
• High school diploma or GED.

Responsibilities

• Perform personal care after assignment and attend to patient requests promptly.
• Provide patient and family with confidential, respectful communication.
• Meet safety needs and use equipment safely.
• Give personal care including baths, back rubs, oral hygiene, and changing bed linens.
• Assist in dressing and undressing patients.
• Assist in feeding as assigned; avoid homemade foods.
• Provide skin care to prevent tissue breakdown.
• Report pain or changes in patient condition to RN Case Manager.
• Assist in ambulation, exercise, ROM, and transfers.
• Assist with self-administered medications as allowed by state regulations.
• Assist with bedpans, urinals, light laundry, and small housekeeping tasks.
• Perform ROM and simple procedures with competency.
• Provide respite for patient/family/caregiver when on-site.
• Keep living area clean and orderly.
• Adhere to documentation and care procedures and standards.
• Participate in QA/PI activities.
• Respect patient and family environment and personal needs.
• As a mandatory reporter, report non-compliance per Dept. of Health within 14 days.
• Report suspected abuse/neglect to proper authorities immediately.
