Job summary

Affinity Health Management is seeking a hospice aide to work under the supervision of a registered nurse, performing personal care and reporting observations to promote comfort and improve quality of patient care per the plan of care.

Responsibilities include assisting with bathing, dressing, feeding, and mobility, while documenting care and maintaining confidentiality. The role emphasizes safety, compassionate communication, and adherence to care protocols in a home or facility setting.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC)
• Current Healthcare Provider CPR certification (hands-on demonstration required)
• Current Food Handlers Permit
• High school diploma or GED
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and providing specialized care to the terminally ill.
• Prior experience in hospice/home health care preferred.
• Basic computer skills to document in EMR.

Responsibilities

• Perform personal care tasks as assigned promptly.
• Communicate with patient and family with empathy and confidentiality.
• Ensure safety using equipment and monitor patient needs.
• Assist with bathing, grooming, feeding, and linen changes.
• Assist in dressing, undressing, and mobility.
• Support with medications reminders where allowed by state law.
• Monitor skin, report pain or changes to RN Case Manager.
• Assist with ambulation and ROM as directed.
• Provide respite care when appropriate.
• Keep living area clean and report documentation per procedures.
