Affinity Health Management is seeking a Hospice Aide to work under the RN supervisor, performing personal care and documenting observations to support a patient’s plan of care. The aide will help maintain comfort and safety and report changes promptly to the Case Manager.

Qualified candidates have CNA training, CPR certification, a high school diploma or GED, and are comfortable working with terminally ill patients. Vaccination against COVID-19 is required per facility guidelines.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• COVID-19 vaccination as per facility guidelines.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Comfortable with hospice philosophy and terminally ill patients.
• Prior home health/hospice experience preferred (1 year).
• Basic computer skills to document in EMR.

Responsabilités

• Provide personal care including baths, grooming, and hygiene.
• Assist with feeding and mobility as directed by the RN Case Manager.
• Ensure patient safety and proper use of equipment.
• Observe and report patient condition changes to the RN Case Manager.
• Maintain patient cleanliness and comfort in a respectful manner.
• Support family with communication and care instructions.
• Document care accurately in EMR as required.
• Adhere to care procedures and professional conduct.
