Job summary

The Pennant Group, Inc. is seeking a Certified Nurse Assistant to join our home health and hospice teams. The hospice aide will work under a registered nurse to provide compassionate personal care and promote patient comfort in the home setting.

Responsibilities include observing patients, reporting changes to the Case Manager, assisting with daily living activities, and maintaining safety standards. A valid CPR certification and ability to work with minimal supervision are required.

Qualifications

• Meets training requirements and competency for hospice care.
• Able to read and follow written instructions and document care.

Responsibilities

• Perform personal care following patient assignments and attend to requests.
• Ensure safety by using equipment properly (foot stools, side rails, O2).
• Provide bath, oral hygiene, shampoo, and linen changes as assigned.
• Assist in dressing, feeding, and nutritional planning.
• Take and record vital signs as ordered.
• Observe skin for breakdown and report changes.
• Support patient mobility and ambulation as instructed.
• Assist with bedpan/urinal and light laundry tasks.
• Document observations and care provided for Case Manager.
