Qualifications

• Active Colorado CNA licensure is required.
• Must be at least 18 years old.
• Ability to read and follow written instructions and document care.
• Self-directed with minimal supervision.
• Empathy for the needs of patients who are ill or frail.
• CPR certification is required and maintained.
• Demonstrates tact, patience and good personal hygiene.
• Licensed driver with a valid auto insurance and reliable vehicle.

Responsibilities

• Provide personal care including bathing, hygiene, grooming, and dressing.
• Assist with toileting, mobility, ROM, and transfers as per plan of care.
• Observe, report, and document patient status and care provided.
• Assist with meals, feeding if needed, and safe food handling.
• Maintain clean, safe patient environment and equipment usage.
• Attend 12 hours of in-service training annually.
• Participate in interdisciplinary group meetings and care conferences.
