Job summary

The Pennant Group, Inc. through Symbii Home Health is seeking a Certified Nursing Assistant (CNA) to support home health care teams under RN supervision, perform personal care, observe and report patient conditions, document care, and participate in IDG meetings.

Requirements include completion of a state-approved CNA training and competency program, minimum 18 years old, CPR certification, valid driver’s license and access to reliable transportation; must comply with state nurse practice act.

Qualifications

• Must have completed a state-approved CNA training and competency evaluation program.
• Minimum age 18 years.
• Ability to read and follow written instructions and document care.
• Empathy for ill, injured, frail and impaired.
• Possess and maintain CPR certification.
• Must have and maintain valid driver's license and access to transportation.

Responsibilities

• Provide personal care (baths, oral hygiene, dressing) and help with daily living activities.
• Observe patients and report changes to the supervising nurse.
• Document care provided and observations accurately.
• Assist with meals, nutrition, and basic comfort measures.
• Maintain clean and safe patient environments.
• Participate in interdisciplinary group meetings and care planning.
