Job summary

Aurora Health Care is seeking a Home Health Aide to provide personalized in-home care across Wisconsin. The role involves assisting patients with daily living activities, vital signs, meal support, and light housekeeping as directed by a registered nurse and care plan.

Candidates must be a Wisconsin Nurse Aide in good standing, have or obtain BLS within 6 months, and hold a valid driver's license. Prior home health experience preferred.

Qualifications

• Nurse Aide in good standing with the Wisconsin Nurse Aide Registry and eligible to work in home health agencies.
• Basic Life Support (BLS) certification within 6 months, unless exempted by leadership.
• Valid driver’s license (WI or IL) required.

Responsibilities

• Make home visits and provide personal care per the plan of care.
• Perform routine personal care activities: bathing, dressing, skin, nails, oral hygiene, etc.
• Take and document temperature, pulse, respiration, and fluid intake/output.
• Assist with transfers and range-of-motion exercises; use bedpan/urinal or commode.
• Assist with meals, feeding, and basic urine testing for sugar/acetone.
• Support home management tasks and light housekeeping related to care.
• Communicate with physicians and care team; document patient information timely.
• Demonstrate professionalism and respect for patients and families.
