Job summary
Advocate Health in Wisconsin seeks a dedicated Home Health Aide to deliver compassionate, patient-centered care in patients' homes under a nurse plan. This full-time role requires 40 hours per week with weekday hours and occasional weekend/holiday coverage.
You will assist with daily living activities, monitor vitals, document health information, and coordinate with the care team. Requires Wisconsin Nurse Aide registry, BLS, reliable transportation, and strong communication.
Qualifications
- Nurse Aide registered in Wisconsin or eligible for Home Health Aide testing.
- BLS for Healthcare Providers certification within 6 months.
- Valid driver’s license (WI or IL).
Responsibilities
- Make home visits and provide personal care per plan.
- Assist with bathing, dressing, skin, nail care and wound care.
- Record vital signs and fluid intake/output; document health info.
- Assist transfers and positioning; use bedpan/commode.
- Assist meals and urine glucose testing.
- Support activities of daily living and home management tasks.
- Communicate with physicians and health team; participate in chart audits.
