Advocate Health is seeking a dedicated Home Health Aide to deliver compassionate patient care in home settings in Wisconsin. You will perform essential personal care tasks, monitor patient status, document health information, and coordinate with nurses and physicians to ensure high-quality care.

This role requires patient, reliable, and compliant individuals with a valid driver’s license and registry eligibility.

Qualifications

• Must have Wisconsin Nurse Aide/Home Health Aide registry listing, or be eligible for competency testing.
• Basic Life Support (BLS) for Healthcare Providers certification to be obtained within 6 months unless exempt.
• Valid drivers license (WI or IL) and reliable vehicle.
• Advanced training beyond high school includes Nursing Assistant program completion.
• Typically requires 1 year of patient care in home health settings.
• Excellent communication and listening skills.
• Must demonstrate ability to provide age-appropriate care.

Responsibilities

• Make home visits to patients and provide personal care per the plan of care developed by the registered nurse.
• Assist with bathing, dressing, skin, nails, oral hygiene and simple wound care.
• Record and communicate patient health information and vital signs.
• Aid transfers, mobility, and positioning; assist with meals and urine sugar checks when applicable.
• Provide home management tasks such as light housekeeping and shopping for groceries/medication when needed.
• Communicate with physicians and care team; participate in chart audits and quality improvement.
