Job summary

Aurora Health Care in Wisconsin seeks a dedicated Home Health Aide to provide personal care and support to patients in their homes under a registered nurse's plan. You will perform routine tasks, monitor vitals, assist with mobility, meals, and basic medical observations while maintaining dignity and comfort.

You must be a qualified Nurse Aide, licensed in Wisconsin, with a valid driver’s license, BLS certification, and a willingness to travel.

Qualifications

• Nurse Aide in good standing with Wisconsin Nurse Aide Registry.
• BLS for Healthcare Providers certification within 6 months.
• Valid drivers license (Wisconsin or Illinois).
• Experience in home health care preferred.

Responsibilities

• Make home visits and provide personal care per plan of care.
• Assist with bathing, dressing, grooming, and basic wound care.
• Record and report vitals and fluid intake/output.
• Assist transfers, positioning, and use of bedpan/urinal.
• Help with meals and testing urine for sugar/acetone.
• Document health information and communicate with health team.
• Participate in chart audits and quality improvement.
• Provide age-appropriate care across lifespan.
• Travel as required; adhere to safety protocols.
