Responsibilities

• Promotes patients to reach functional status goals by assisting with ADL's following HHA assignment record and maintaining safety.
• Notes changes in patient's condition and informs the case manager or RN/Supervisor and documents accordingly.
• Performs personal care, feeding, laundry, light room cleaning and washing as assigned.
• Maintains patient safety using equipment properly and documents care as required.
