Job summary

Advocate Health is hiring a Home Health Aide to visit patients in their homes and facility settings in Wisconsin. The role requires providing personal care, assisting with activities of daily living, and supporting clinical staff under a registered nurse's plan of care.

The ideal candidate will have a Nurse Aide certification, BLS within 6 months if required, and a valid driver's license with auto insurance. Travel to multiple sites is expected as part of the role.

Qualifications

• Nurse Aide in good standing with the Wisconsin Nurse Aide Registry and eligible to work in home health agencies and hospices.
• BLS for Healthcare Providers certification to be obtained within 6 months unless exempt.
• Must have a valid driver’s license and vehicle insurance in the state of WI or IL.

Responsibilities

• Makes home visits to specified patients and provides personal care according to the plan of care.
• Performs routine personal care activities including bathing, dressing, grooming, basic wound care, and catheter care.
• Records vital signs and fluid intake/output; documents health information.
• Assists with transfers, mobility, meals, and urinal/bedpan use.
• Applies plan of care, assists with meal planning and testing of urine for sugar and acetone.
• Provides home management tasks and light household duties as appropriate.
• Communicates with physicians and health care team; participates in chart audits and quality improvement.
