Job summary

CenterWell Home Health seeks a Speech-Language Pathologist to deliver home-based SLP services, evaluate patient needs, and coordinate care with clinical leadership. You will conduct assessments, develop care plans, and support patients in communication, swallowing and cognitive goals to improve functional independence.

The role requires CCC-SLP or Audiology, at least six months of experience, current license, CPR, and reliable transport.

Qualifications

• Certification in Speech-Language Pathology or Audiology (ASHA).
• Minimum six months of experience as a speech therapist/pathologist.
• Current, unrestricted professional license and CPR certification.

Responsibilities

• Evaluate and provide speech-language pathology services in home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess communication, voice, language, swallowing and related disorders.
• Administer and interpret diagnostic tests and therapeutic techniques.
• Document patient progress and coordinate care with supervisor/physician.
• Educate patients and families on use of alternative communication systems.
• Discharge planning and care coordination participation.
