Job summary

CenterWell Home Health is seeking a licensed Speech Language Pathologist to evaluate, direct and provide speech-language pathology services to patients in the home or facility, reporting to the Clinical Manager. You will participate in Plan of Treatment and Plan of Care development, assess and treat disorders of speech, voice, language, hearing and swallowing, and document progress with fidelity.

This role requires ASHA CCC-SLP/CCC-A, minimum six months of experience, current license, CPR, and a

Qualifications

• CCC-SLP/CCC-A required by ASHA.
• Minimum six months experience as a speech therapist / SLP.
• Current and unrestricted license; CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction to patients and families; maintain documentation per policy.
• Participate in care coordination and discharge planning.
