Job summary

CenterWell Home Health is seeking a licensed Speech-Language Pathologist to deliver in-home therapy, focusing on speech, language, voice, swallowing, and functional rehabilitation. You will evaluate patients, develop care plans, and coordinate with physicians and staff to improve independence and quality of life.

Applicants should have at least six months of home health experience, CPR, a valid driver’s license, and reliable transportation.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in the patient’s condition to supervisor and/or physician.
• Provide instruction and training to patients in use of alternative communication systems.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation and reports as required by Company policy.
• Participate in care coordination activities and discharge planning.
• Maintain confidentiality of patient information.
