Job summary

CenterWell Home Health is seeking a speech-language pathologist to deliver in-home therapy focused on rehabilitation and functional improvement. You will evaluate, treat, and coordinate care for patients in their homes, and work with families and staff to promote independence.

The role requires ASHA CCC or eligibility, current license, CPR, and at least six months of home health experience. A valid driver's license and reliable transportation are required.

Qualifications

• Meet the experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum of six months experience as a speech therapist or speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Good organizational and communication skills.
• A valid driver’s license and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to minimize impact.
• Administer and interpret diagnostic tests and apply therapies.
• Record and report changes in patient condition to supervisor/physician.
• Provide instruction to patients in use of alternative communication systems when appropriate.
• Counsel patients, families and health care staff.
• Prepare timely written reports of evaluations, visits, care plans and progress.
• Participate in care coordination and discharge planning.
