Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate, direct, and provide speech-language pathology services to patients in home or facility settings. You will participate in building and reviewing the Plan of Treatment and Plan of Care, and apply professional judgment to assess and treat disorders of communication and swallowing.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology or Audiology (ASHA).
• Minimum 6 months experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver’s license with reliable transportation and auto insurance.

Responsibilities

• Evaluate, direct and provide speech/language pathology services in home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapy.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain documentation and submit reports per policy.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and share information only as required.
