Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in home or facility settings. You will develop care plans, administer tests, and document progress while coordinating with the clinical team to ensure optimal outcomes.

Requirements include CCC-SLP or ASHA certification, at least six months of SLP experience, a valid license and CPR, and a valid driver’s license with reliable transportation for patient visits.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license and CPR certification.
• Valid driver's license with auto insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology services in home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic treatments.
• Maintain documentation: evaluations, visits, care plans and progress reports.
• Provide instruction to patients and families; participate in discharge planning.

ASHA CCC / Clinical license
