Job summary

Care Partners Home Health, part of the Optum family, is hiring a Physical Therapist Assistant for a part-time role in the Wetzel, Tyler, Marshall, and Ohio Counties service area. You will deliver treatment under the supervision of a Physical Therapist, educate patients and families, and help patients improve mobility and function in a home-care setting.

The position emphasizes adherence to state practice laws, patient safety, and collaboration with the care team.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state laws governing the provision of physical therapy in home care and adhere to the supervising PT's plan.
• Instructs and aids patients in active and passive exercise, muscle re-education, gait, ADL, transfer, safety, and prosthetic training.
• Observes, records, and reports to the supervising PT, nurse supervisor, and/or physician the patient's response and changes in condition; coordinates care with the care team.
• Instructs patient, family, caregiver, and other health care team members in therapy within PTA scope.
• Function in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
