Job summary

CenterWell Home Health, a Humana subsidiary, seeks a Home Health Physical Therapist Assistant to deliver in-home PT services under a PT plan. The role reports to the Clinical Manager and focuses on functional gains in geriatric patients in patients' homes.

Responsibilities include administering therapies, documenting records weekly, and coordinating with the health team while maintaining infection control and safety standards. In-person visits only, full-time position.

Qualifications

• Current state license to practice Physical Therapy.
• Valid CPR certification.
• Experience collaborating with a healthcare team.
• Comprehensive knowledge of geriatric changes.
• Read, write and speak fluent English.

Responsibilities

• Administer the physical therapy plan to patients in their homes as approved by the PT.
• Perform non-complex manual therapy, massage, traction, heat, cold, electrical modalities.
• Document client records and submit weekly to the office.
• Attend educational meetings and departmental in-services.
• Notify PT or Branch Director of equipment issues or staff/client changes.
• Apply infection control and universal precautions.
